The U.S. Department of War has implemented new comprehensive clinical guidance for health and human performance optimization across all military departments, effective immediately, to enhance military readiness.
This guidance, issued by the Director of the Defense Health Agency on September 2, 2026, follows a July 15, 2026, memorandum from the Secretary of War. The Secretary's directive mandated testosterone deficiency screenings for all Service members aged 30 and older during their Periodic Health Assessments, with optional screenings available for those under 30.
The new clinical guidance also addresses female hormonal optimization and establishes uniform screening and treatment pathways throughout the Defense Health Agency enterprise. It further ensures the availability of clinical training for all primary care providers and assigns responsibility for compliance.
By proactively identifying and treating suboptimal hormone levels, the Department aims to continue investing in the health of its warfighters, strengthen their performance, and maximize force readiness.
