Johnny Lee Underwood, 27, of Mabelvale, has been sentenced to 20 years in federal prison for distributing fentanyl that resulted in a death.

United States District Judge Brian S. Miller handed down the 240-month sentence on Tuesday, May 19, 2026, which also includes three years of supervised release. Underwood pleaded guilty on February 4, 2026, to one count of distribution of fentanyl resulting in death, following an indictment on August 1, 2023.

The case began on June 30, 2023, when Little Rock Police responded to an overdose, finding a victim who had ingested a blue pill containing fentanyl. An investigation by the Federal Bureau of Investigation (FBI) revealed Underwood sold approximately four counterfeit oxycodone pills, containing fentanyl, to the victim and their girlfriend. The victim died hours after ingesting a portion of a pill, with an autopsy confirming fentanyl toxicity as the cause.

Jonathan D. Ross, United States Attorney for the Eastern District of Arkansas, emphasized the lethality of fentanyl, stating, "Sadly, here is yet another proof that 'One pill can kill'." He added that his office would continue to hold accountable those who knowingly sell fentanyl leading to death. Jason Van Goor, Special Agent in Charge of the FBI Little Rock Field Office, reiterated the FBI's commitment to removing dangerous drugs and pursuing individuals who profit from putting lives at risk.

The investigation was a collaborative effort between the FBI and the Little Rock Police Department, with Assistant United States Attorney Amanda Fields prosecuting the case.
